Hello and welcome to the Slimmr online consultation! We’re very excited to help you on your weight loss journey and thank you for choosing us. This consultation form is carefully designed to ensure we have all the information we need to prescribe safely. Over the following pages it will cover things like your weight, height, past medical history, your current medication and allergies. We’ve tried not to make it too long and boring but the questions we do ask are important. Weight loss medication isn’t suitable for everyone so it’s important you answer these questions as honestly and accurately as possible. The safety and appropriateness of the treatment will be a joint decision between yourself and the prescriber.

Not all patients are eligible for our programme. If you're not eligible for the programme, then we’re really sorry. This is for your own health and safety. We recommend you discuss weight loss options with your GP or another healthcare professional. We aren’t allowed to give you a detailed reason why as this would mean people could return and change their answers.

Before you start the form, it's important to realise that in order to prescribe safely we must check your Summary Care Record. We do this automatically after we've submitted the form. This is a brief record of your medication and medical history that is accessible to pharmacies.

This consultation form is for both Wegovy and Mounjaro. Towards the end of the consultation, you’ll be asked which medication you would like to take. Don’t worry if you are unsure – there is an option to request that the prescriber decides for you, based on the information that you have provided.
