This form will ask how you feel on your current dose, any side effects and will ask you to check your weight. It will also ask you if you are happy to increase the dose, as per the titration regime.
The completed form will then be reviewed by one of our prescribers and if it is safe and appropriate to do so they will prescribe your next pen. If they have any questions they will call you directly.

Simple steps that may help include rotating injection sites, keeping the skin clean and dry before injecting, and avoiding tight clothing that rubs the area. A cool compress can feel soothing. If the reaction is spreading, getting worse, or you're concerned, let your prescriber know. If you have symptoms that feel severe or widespread (for example swelling of the face/lips, wheezing, or difficulty breathing), seek urgent medical help.
Bloating and wind can be more noticeable when your appetite and digestion change. Many people find it helps to eat more slowly, choose smaller portions, and avoid fizzy drinks if they trigger symptoms. Sipping drinks rather than gulping can also help. If symptoms are persistent, worsening, or affecting your day-to-day, mention it to your prescriber at review.
Constipation can occur, particularly early on. Some people find it helps to keep fluids up, include regular gentle movement, and aim for consistent meals with plenty of fibre-containing foods (in a way that feels manageable for you). If constipation lasts more than a few days, is painful, or you're unsure what's safe to use alongside your medication, speak to a pharmacist or your prescriber before trying remedies.
If you have diarrhoea, it's important to avoid dehydration. Small, frequent sips of water can help, and some people find lighter meals are easier to tolerate for a short period. If caffeine makes symptoms worse, it may help to reduce it while symptoms settle. If diarrhoea is severe, persistent, or you have signs of dehydration (for example dizziness, very dark urine, or not passing urine), contact your prescriber for advice.
Dizziness can happen for a few different reasons, including dehydration or not eating enough. Try to take regular fluids, eat consistently, and stand up slowly from sitting or lying down. If you notice dizziness after missed meals, it may help to keep meals and snacks more regular. If dizziness is frequent, worsening, or you feel faint, contact your prescriber so they can check what's going on.
Feeling more tired than usual can happen, particularly when your appetite changes and you're eating less. Try to keep meals regular, stay hydrated, and build in gentle movement if you can. Prioritising sleep and keeping a steady routine can also help. If fatigue is persistent, affecting your daily life, or feels out of proportion, let your prescriber know.

Hair shedding can sometimes occur during weight loss. Supporting your body with regular meals and enough protein can help, and gentle hair care (avoiding harsh treatments and excessive heat) may reduce breakage. If hair loss is ongoing or worrying, mention it to your prescriber. They may advise checks or guidance based on your overall health and diet.
Headaches can be linked to dehydration, missed meals, or changes in sleep. Small, frequent sips of water and regular meals can help, and some people find rest and reducing screen time helps symptoms settle. If headaches are severe, persistent, or you have any other concerning symptoms, contact your prescriber for advice.
Heartburn can be more noticeable if meals are larger or eaten quickly. Some people find smaller, slower meals help, and avoiding lying down straight after eating can reduce symptoms. If certain foods or drinks trigger symptoms (for example spicy foods, high-fat meals, caffeine, fizzy drinks), reducing those triggers may help. If symptoms are frequent, severe, or not improving, discuss it with your prescriber or a pharmacist.

You have selected an enhanced switch dose. This prescribing approach is off-label, meaning it differs from the licensed starting regimen for Wegovy tablets.

If you have reached your goal weight and are now moving to a maintenance dose, the prescription team may call to check on progress. Please note that due to the regulations for this medication, they may not be able to finalise the prescription until they have spoken to you.
Your prescriber will contact you directly to discuss your next dose. We'll set your order to stay on the same dose until your prescriber decides otherwise.
Due to the gap since your last Slimmr prescription, you’ll need to restart the programme and complete the required checks before any further medication can be issued.
If you will be away for an extended period, you can request enough medication to cover your trip. This usually means two pens of your current dose.
We normally provide one month's supply at a time, so travel requests are reviewed by a prescriber and approved where appropriate.
